PHOTO IDENTIFICATION

The Hornell Police Department provides Photo ID cards to the community. The fee for the card is $5.00

The process takes approximately 5 minutes. Identification is needed to obtain a Photo ID

Some examples are:

• Old drivers license or permit
• Social Security Card
• Student ID
• Health Insurance Card
• Medicaid ID
• Supermarket Check Cashing card
• Birth Certificate
• High School Diploma
• Mail or W2s proving your address and Date of Birth
• Passport
• Military ID
• Anything that will help prove your identity and address.
